Highlight on Diabetes

The AAFP Foundation and the Family Medicine Philanthropic Consortium (FMPC) are pleased to announce the Highlight on Diabetes grant program, designed to improve the management of diabetes by providing educational resources for family physicians and the patients they serve. 

To apply, please review and complete the Grant Guidelines and Application (2-page PDF file; About PDFs).
The AAFP Foundation will provide a one-time $5,000 grant to AAFP Constituent Chapters or Chapter Foundations. The grant must be used to present an exhibit and/or a non-accredited panel discussion on the management of diabetes and to support other meeting-related expenses.

In addition, grant recipients will receive the customizable Highlight on Diabetes Meeting Resource Kit (1-page PDF file; About PDFs), developed by the AAFP Foundation and a family physician consultant. The kit will provide all materials needed for implementation at your annual Chapter meeting (or other approved event).
Highlight on Diabetes Meeting Resource Kit -- The AAFP Foundation will provide a customizable Highlight on Diabetes Meeting Resource Kit at no additional cost to support the exhibit and/or panel discussion. The customizable Meeting Resource Kit was developed by the AAFP Foundation and a family physician consultant.
