Family Medicine Cares Grant Recipient

The AAFP Foundation was proud to provide Faithworks Medical with a Family Medicine Cares USA grant. The clinic held a ribbon cutting ceremony on June 28, 2012 and officially began seeing patients on July 10th.

Faithworks Medical is being led by AAFP member Dr. Susan Hulsemann, who runs a family medicine practice in Monroe and is the director of the new family medicine residency program developed at Mercy Memorial Hospital.

Clinic Update

The clinic held a ribbon cutting ceremony on June 28th and officially began seeing patients on July 10th, 2012.

  • Total Patients: 52 (the clinic is seeing eight to 10 patients per week)
  • Total Volunteer Hours: 45
  • Number of Licensed Health Care Providers: 2
  • Family Physician Volunteer Hours: 45
  • Value of Services: $9,169
