AAFP Healthy Interventions: Tobacco and Nicotine

Program Overview

The AAFP Foundation provides financial support to AAFP Healthy Interventions. The Healthy Interventions framework is a comprehensive, integrated approach to promote healthy lifestyles and behaviors to assist AAFP members with their patients and practices. The Healthy Interventions activities are focused on providing solutions that will help patients strive for health, including office-based tools, community engagement, advocacy, and evidence-based knowledge.

About Healthy Interventions

This initiative empowers AAFP members with knowledge and tools to address health equity and social determinants of health that impact their patients and communities in striving for healthy lifestyles. This strategy encompasses the following elements:

  • Improving the health of populations within the context of the PCMH.
  • Improving health by encouraging healthier lifestyles in the entire population, including increased physical activity, better nutrition, avoidance of behavioral risks, and wider use of preventive care.
  • Family physicians are central to population health improvements including: providing evidence-based clinical care (i.e. Choosing Wisely); ensuring a fully engaged and activated patient; and care coordination.
  • Addressing primary care and public health/community integration.
