Family Medicine Chapter Alliance Grant Awards
Supporting the philanthropic work of AAFP Constituent Chapters and Chapter Foundations since 2006.
How to apply
We encourage all Chapter/Chapter Foundations to apply who are committed to supporting philanthropic projects focused on health priority areas and reflect the rich diversity of the specialty and the patients served.
The application period is open from January 1 to February 28. Apply through the online portal by clicking on the Apply Today button below. Note: The length of the application, budget and all attachments are limited to a total of 12 pages.

- Applicant must be a Chapter or Chapter Foundation of the American Academy of Family Physicians and a 501(c)(3) organization or a 501(c)(6) organization.
- Funding will only be made to support philanthropic activities.
- Grants will not be made to individuals or endowment funds. Grants will not be made to support political campaigns or the lobbying of any public official, or for Chapters or Chapter foundations to accomplish fundraising. Grant applications that fail to comply with all eligibility and submission requirements will not be considered for funding.

- Proposed projects should align with one of the following FMCA priority areas:
- Student and/or Resident project: Focuses on education, research, and/or humanitarian opportunities for students (high school, undergraduate, or medical students) and/or Family Medicine residents.
- Member Outreach project: Focuses on education, research, and/or humanitarian opportunities that target AAFP Family Physician members.
- Public Health project: Focuses on the health of families and communities by promoting healthy lifestyles; conducting research and/or providing education for disease and injury prevention; and/or projects that are humanitarian or service in nature that improve health of individuals or communities.
- Complete the budget requirement by downloading the Budget Worksheet and uploading the completed form with your application.
- FMCA funding request must be at least 70% for program services with a maximum of 30% for staff and administration. (For example, if total FMCA request is $2,000, staffing and administration cannot exceed $2,000*.30=$600.)
- Submit a final report upon the completion of the project period, due no later than August 15 in the year after the award was granted.
- A no-cost extension of the final report may be granted. Extension requests must be emailed with a proposed due date to Ashlyn Ewing by July 15.

How many applications can I submit?
Each state is limited to submitting a total of three (3) grant applications per year, but only one (1) application per project. Applications can be submitted by the Chapter, Chapter Foundation, or a combination of the Chapter and/or Foundation.
Do you accept requests for continued funding for same/similar projects?
Yes, there is no limit on the number of times that a grant project can receive funding from the FMCA.
Will I receive the full amount requested in the application?
Applications are scored by reviewers from the FMCA Steering Committee and funding is awarded based on the average of 3 scores. Each cycle, only one top-scoring application will receive full funding with remaining applications receiving funding based on score, number of applications received, and funding available. The Steering Committee strives to provide funding for the largest number of applications.
Can I request funding for a project under both the FMCA Grant Awards and the Chapter Engagement Grants?
Yes, you may apply for funding from both the FMCA Grant Awards and the Chapter Engagement Grants for the same project.
Can I include stipends in the requested funding?
Yes, you may include stipends in your budget. But please be aware that administrative costs (for staff, personnel, interns, consultants, volunteers, etc.) CANNOT exceed 30%* of your total request from FMCA.
*For example, if total FMCA Request is $7,000, FMCA Staff & Admin cannot exceed $7,000 *.30=$2,100.
