Outstanding Program Award
Recognizing the exceptional programming of AAFP Chapters and Chapter Foundations.
The AAFP Foundation established this award to nationally recognize outstanding philanthropic activities of AAFP Chapters and Chapter Foundations. Each year, one program is selected, and the Award is presented during AAFP FMX at the AAFP Foundation recognition event.
The Outstanding Program Award Recipient will receive up to a $1,000 reimbursement for travel to FMX to accept the award at the AAFP Foundation Donor Recognition dinner.
Nominee must be a philanthropic program/project of a Chapter or Chapter Foundation of the American Academy of Family Physicians and a 501(c)(3) organization or a 501(c)(6) organization.
All AAFP Constituent Chapters and Chapter Foundations are eligible to submit nominations, either to recognize one of their own programs, or that of another Chapter/Chapter Foundation.
Each Constituent Chapter may have one nomination per year. If a state has both a Chapter and Chapter Foundation, only one nomination is permitted.
Non-winning programs may be re-submitted for nomination each year with updated information.
OPA recipients may not submit a nomination for the same program for a period of six years.
The philanthropic program or project submitted can be either programming or development related.

To nominate a Chapter/Chapter Foundation Program
A typed Recommendation Letter, signed by the Chapter Executive (or equivalent staff person) and/or the President of the Chapter or Chapter Foundation.
Letter will be reviewed in order to access the program’s content, presentation, creativity, measurable outcomes, and relationship of the program to the mission of the AAFP Foundation
Names of two representatives of the winning Chapter or Chapter Foundation who will be invited to attend a recognition event held during AAFP FMX as honored guests of the Foundation.
